Most manual handling training focuses on avoiding the “slipped disc”. Yet this concentrates on something that is relatively uncommon, while overlooking one of the genuinely frequent causes of mechanical low back pain: damage to the vertebral endplate.

The overlooked cause of back pain

The primary cause of loss of function in many cases of mechanical low back pain is the fracture of a structure called the vertebral endplate — the thin layer between the disc and the vertebra above or below it. Because these fractures are often very small, they can be difficult or impossible to see on a standard X-ray, so they are frequently missed.

Why small fractures matter

An initial endplate fracture may be painless, which is part of what makes it so easy to overlook. But even a minor fracture can depressurise the disc, changing how it bears load. Over time this can drive further damage, contributing to conditions such as disc degeneration, osteoarthritis and, eventually, disc herniation. In other words, what looks like a sudden “slipped disc” years later may have begun with a small, unnoticed injury much earlier.

Consequences of a broken endplate

Once an endplate is damaged, the affected disc no longer manages pressure as it should. The surrounding structures take on more strain, and a cascade of wear and degeneration can follow. Understanding this helps explain why prevention — rather than simply reacting to obvious injuries — is so important.
